Only those players actually competing in the tournament will be eligible for awards or prizes.p> <pfont size=3 face="Times New Roman">C. span>font>span>fontfont size=3>If a selected team is unable to attend the Tournament the Tournament Registrar must be notified immediately by telephone. This shall be followed by written or fax confirmation prior to February 15, 2005. Failure to do so will result in forfeiture of the application fee.span>b>span>font>p> <pbfont size=3 face="Times New Roman">1.1 span>font>span>font>bbfont size=3>Team Roster and Player/Team Official Registration Formspan>font>b>p> A. span>font>span>font>b>A photocopy of each 2004-2005 C.H.A Registration Card or C.H.A. approved team registry, U.S. Hockey, I.I.H.F. registration forms, or approved Regional rosters for uncarded teams, must accompany and be attached to the team registration form. Affiliates and casual players must be clearly marked on the roster.p> 1.2 span>font>span>font>bb>Letter of Permissionspan>b>p> <pfont size=3 face="Times New Roman">A. span>font>span>fontfont size=3>Each team must have the “Letter of Permission”, or “travel warrant” completed by its home Branch or Minor Hockey Committee, which agrees to impose and enforce any disciplinary action necessary against the team, its players or officials, as per C.H.A. Reg.
